2 dental health visits (or perio maintenance visits for Gold and Platinum Plans) per plan year
2 problem-focused exams per plan year
4 bitewing X-ray films per plan year
1 full mouth X-ray (usually taken every 3-5 years)
2 fluoride treatments per plan year
Program Guidelines
There will be a $50.00 reinstatement fee if your plan lapses beyond the re-enrollment date.
Plan runs year-to-date from the initial purchase date.
Cannot be used in conjunction with another dental plan or discount.
NON-REFUNDABLE: No refunds or premiums will be issued for any reason. The participants are responsible for utilizing the plan during the coverage date and no extensions shall be given.
Patient’s portion of the bill is due on the day of service.
The Dual Plan can be used for parent/guardian and child OR for two partners in the same household.
The Family Plan includes family members and children who are enrolled full-time in college until age 23, or children who are not enrolled full-time in college until age 18.
This plan is only honored at Manlius Dental Group.
